Yeah its leaving the state before it gets to the action, that's why it isn't getting done. If you want everything in the state to get done before leaving the state, then setup the last action to finish to fire the event, or use the Next Frame Event action so everything has a whole frame to process.
More likely, the first one is the same way, but the second one is processed fast enough to get done. I'm not positive, but timing issues are probably the most common 'gotcha' problem in Playmaker logic flows so make sure you check that sort of thing when weirdness starts happening.
